From this page, you can download forms that you may need to submit to the Office of Correspondence Studies as you progress through your correspondence study course.

Course Materials Order Form
If you order your course materials from Ed Map by mail or by fax, please fill out and submit the
Ed Map Order Form.
Change Name, Address, Telephone, or E-Mail
If your personal information changes while you are enrolled in a course, please submit the
Change Personal Information Form.
Request to Drop
If you wish to drop a course, you may submit a
Request to Drop a Course.
Enrollment Extension
To request an enrollment extension, complete
the
Extend Enrollment Form and submit this form, along with the
$35 extension fee, to our office.
Permission to Release Grades
If you submit a signed
Permission to Release Grades form
allowing our office staff to do so, you may call our office, and our
staff will tell you your grade over the phone.
Exam Request Form
Follow the instructions in your study guide for submitting the
Exam Request Form, enlisting a proctor, and scheduling an exam.
Assignment cover sheet
When submitting an assignment by mail or fax, be sure to attach an
Assignment Cover Sheet to the assignment.
