To document and report student misconduct, or to terminate a student’s enrollment in a faculty-led Study Abroad Program, download and complete the
Student Misconduct Reports. This Word document includes two forms, the Initial Student Misconduct Report in a Faculty-led Study Abroad Program and the Termination of Student from a Faculty-led Study Abroad Program. The first form is a way to document and provide a warning to students whose behavior is deemed inappropriate by the Academic Program Director. Use the second form if termination of a student is necessary.
The first form must be printed out and signed by the Academic Program Director, witness(es), and the student. With the second form, include documentation supporting the decision to terminate the student’s participation. Copies of the completed documents and all supporting documentation may be scanned and e-mailed as PDF attachments, faxed, or mailed to the Office of Study Abroad Programs. Please retain the originals, and submit them to the Study Abroad Office upon your return to campus.
